SUMMARY CARE RECORDS 

 

There is a Central NHS Computer System called the Summary Care Record (SCR).  The SCR has been introduced to help emergency doctors, nurses and out-of-hours services help you when you contact them when the Surgery is closed.  Initially, it will contain just your medication and allergies and adverse reactions.  Later, as the SCR develops, other staff who work in the NHS may be able to access it along with wider information about your health, for example hospital and specialists' letters.

 

Information about your health will be extracted from your Practice records and held on a central NHS database.  As with all systems there are things for you to think about.  When you speak to an emergency doctor you might overlook something that is important and if they have access to your medical record it might avoid mistakes or problems.  You should be asked to give your consent each time a member of NHS staff wishes to access your record, unless you are medically unable to do so.  On the other hand, you may have strong views about sharing your personal information and wish to keep your information at Practice level.

 

The Health & Social Care Information Centre (HSCIC) who are responsible for the SCR have agreed with doctors' leaders that new patients registering with this Practice should be able to decide whether or not their information is uploaded to the Central NHS Computer System.  Existing patients have also been offered the option to opt out.

 

The doctors at Haddenham Surgery believe that it is correct to sign up to the SCR and our system supplier has all the security processes in place to protect your personal and medical information.  In January 2015 the practice signed up to have patient records uploaded on the "spine".

 

If you do not wish your records to be uploaded to the "spine", please complete an opt-out form and return it to Reception.  Once we have received a signed opt-out form, we will code your record to exclude it from the upload.
