If you wish to register for these online services complete the consent form below then bring the completed form to reception along with photographic proof of identity, so that an account can be created for you. (Please Note: Due to Data Protection requirements we need to see either a photo ID driving license or current passport to register you for on-line services)
Online services are offered to patients over 16 years, however if you regularly request medication on behalf of someone else, please contact the surgery to discuss proxy online access.
Once you have registered for your online account. The obsure password we send you will need to be changed to something memorable to you.
If you have previously registered for Online Services, but have forgotten your password, please contact the surgery to have your password reset.
