Care and Support

Care and support is the mixture of practical and emotional support required to help people manage their lives and be independent – including people with a new cancer diagnosis, older people, people with a disability or long-term illness, people with mental health problems and carers.  At Haddenham Surgery, we offer care and support as well as medical care to our patients.

We offer initial review and assessment at diagnosis and annually thereafter for our patients with long-term conditions.

Whilst we may be unable to fulfil all the needs of our patients, we will try to help and guide you to other services that may be better placed to offer the required support.
