ALZHEIMER’S SOCIETY and CARING TOGETHER
There is a new service which offers intensive support available to carers, over a two-to-four-week period either to help prevent the person they care for from being admitted into hospital (due to effects of delirium), or before they are discharged from hospital, (should they have been diagnosed while in hospital). Alzheimer’s Society and Caring Together have created a helpful flyer which includes what support is available; what happens next; who will provide support; and contact details for carers wishing to make a referral.
