Annual Review for Long Term Conditions (LTC)

We are introducing a new recall system, so your annual review will now be due in the month of your birth.

It is our plan to do all of your tests at one appointment and only arrange a second appointment if the clinician feels that it is necessary.  Although this may mean that your appointment will be a little longer, hopefully the number of appointments you need in the long run, will be less.

It is important to attend for your annual review appointment so that we can ensure your condition is properly controlled and managed.  If you run out of your repeat medication and have not had a review, it could cause a delay to re-authorising your repeat medication.

We will  contact you, either by email, SMS, letter or telephone to ask you to make your annual review appointment.  The time allocated for your review will be different for each patient.

How long will your appointment be for:

  • Asthma ONLY then it’s a 10 minute appointment
  • COPD ONLY then it’s a 10 minute appointment
  • Asthma PLUS COPD then it’s a 20 minute appointment
  • Hypertension OR Heart disease OR Kidney disease then it’s a 20 minute appointment
  • Asthma PLUS either Hypertension OR Heart disease OR Kidney disease then it’s a 30 minute appointment
  • COPD PLUS either Hypertension OR Heart disease OR Kidney disease then it’s a 30 minute appointment
  • DIABETES PLUS either Hypertension OR Heart disease OR Kidney disease then it’s a 30 minute appointment

What to expect:

  • We will ask about any symptoms — so keep a note of them before you come,
  • We may do routine blood tests,
  • Please bring any inhalers you use,
  • Please bring an early morning specimen of urine.

DIABETES – your appointment will be for blood tests, blood pressure, foot check and general review.  It would help if you are dressed appropriately for this appointment.  The clinician will review your blood results and arrange a follow up appointment to review your diabetes only if they feel it is necessary.

HEART DISEASE – your appointment will be for blood tests, blood pressure and general review.  It would help if you are dressed appropriately for this appointment.  The clinician will review your blood results and arrange a follow up appointment only if they feel it is necessary.

ASTHMA and/or COPD – your appointment will be for assessment of inhaler medication and technique and general respiratory review.  The clinician will arrange a follow up appointment only if they feel it is necessary.  It would be helpful if you could complete the ASTHMA QUESTIONNAIRE before your visit.

OTHER CONDITIONS – your appointment will be for any necessary blood tests and general review.

CHILDREN (Under 14 years) – children under 14 years with long term conditions are likely to be seen at hospital for annual review.  If your child has not been seen within the past twelve months, please contact the surgery so that we can take appropriate action.  Children with ASTHMA, that are not being reviewed at hospital, should make an appointment at the surgery.

It is anticipated that it will take up to two years for this new system to be fully adopted and patients may require more than one appointment to bring their annual review in line with their birth month.
