Asthma can change over time so it is important that you are reviewed annually – usually around your birthday.
The review is to make sure that you are taking the right medicines and in the right amounts. It is essential you have a good inhaler technique to ensure that medication gets to the part of your lungs that need it. So before you start the Questionnaire, please watch the video for the inhaler you use, from this list of Asthma UK Inhaler Videos.
Patients registered with Haddenham Surgery can complete the Asthma Questionnaire – (1) using SystmOnline (top button) or if you do not have access to this, (2) use the website Questionnaire (second button). (3) If you are unable to complete either of these Questionnaires – please contact the surgery and one of the team will make alternative arrangements for you.
Once you have completed the questionnaire, click “SUBMIT”. The Practice Nurse will review your answers and your current asthma medicines. One of the Respiratory Team will then telephone you to discuss your results, so please make sure we have an up to date telephone number for you.
Many things can affect your asthma over the year – from work to weather, age to stress levels – so do make sure you answer the questions honestly as this will help the team to ensure you are getting the treatment that will suit you best at this time.
The Asthma Control Test is a quick way for you to assess how well your Asthma is controlled. It can be done at any time and as often as you like. There are two tests; one for children 4-11 years and one for everyone over the age of 12. Simply complete the questions, 7 (for children) and 5 (for over 12), then calculate your score.
