People with a learning disability often have poorer physical and mental health than other people. This does not need to be the case.
It is important that everyone over the age of 14 who is on their doctor’s learning disability register has an annual health check.
We will call you once a year for your health check. You can look at the HEALTH CHECK QUESTIONNAIRE to see the kind of things we will discuss.
If you have problems or concerns, you don’t have to wait for your annual review to talk to your GP, contact us in the usual way via AskMyGP.
